Department Description

Our Community Health Nurses deliver care to patients in the community, providing a wide range of skilled nursing care while working to the top of their licensure. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain living in the setting of their choice, their home.

Requirements: 

- Greater than one year of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired

On Call: Not Required

Requirements:

  • Current RN licensure recognized by the State of Vermont required.
  • Appropriate experience in specific clinical area. Varies by unit.
