Social Work Jobs in Elizabethtown, NY at The UVM Health Network can include: Licensed Master Social Worker - Team members in these social work jobs, under the operational Leadership of Case Management, plan, organize administer and coordinate all activities for Social Work hospital-wide. The Licensed Master Social Worker (LMSW) provides professional application of Social Work Theory, principles, and methods to prevent, assess, evaluate, formulate and implement a plan of action based on the clients needs and strengths, and intervene to address mental, social, emotional, behavioral, developmental, and addictive disorders, conditions and disabilities, and of the psychosocial aspects of illness and injury experienced by individuals and society. Clinical Social Worker - Team members in these social work jobs work in collaboration with other members of the health care teams at The UVM Health Network. Work involves evaluation of and support for psychosocial needs of patients and their families. The Clinical Social Worker also assists with serious illness conversations, including exploring sources of suffering, prognostic awareness, patient centered values, information needs, treatment preferences, organizing team/family meetings. Mental Health Clinician Case Manager - Team members in these social work jobs provide direct clinical mental health evaluation and individual and group psychotherapy to the patient population of individuals experiencing a sub-acute or acute mental health crisis. The Mental Health Clinician Case Manager is also responsible for formulating the plan of care and coordinating treatment for the patient while in the program.
