Position Summary:
We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our team at Helen Porter, a 120-bed facility specializing in long-term care and rehabilitation. This role offers a consistent part-time schedule with opportunities for additional shifts and future growth to full-time based on patient census and care needs.
Key Responsibilities:
- Provide high-quality care to a diverse patient population, primarily long-term care residents with some rehabilitation patients.
- Collaborate with a multidisciplinary team including physicians, nursing staff, and administrative leaders.
- Conduct patient assessments, develop care plans, and manage ongoing treatment.
- Document care using Epic EMR (training provided).
- Maintain compliance with facility protocols and regulatory standards.
Schedule & Coverage:
- Standard Schedule: 2 set days per week (Wednesday and Friday preferred).
- Shift Length: 10-8-hour shifts
- Coverage Area: 120-bed facility (primarily long-term care).
- Team Structure: 2 physicians + 1 full-time APP (typically 2 providers onsite daily).
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Vermont.
- Experience in long-term care, geriatrics, or rehabilitation preferred.
- Strong communication and collaboration skills.
- Ability to work independently and as part of a team.
