JOB DESCRIPTION:

The Regional Transfer Center (RTC) Nurse Coordinator has several roles in assuring the safe and timely coordination of care for patients across the University of Vermont Health Network and region.
The Level 2 Nurse Coordinator successfully acts in at least two both of the following roles:
Transfer Role: He/she fields calls from physicians who are seeking support in coordination of care for patients including but not limited to Telehealth services, phone consultations, transfer to alternate facilities for specialty services or higher level of care, or procedure coordination. The nurse will coordinate emergent, urgent or specialty treatment; apply clinical expertise to prioritize admissions; work with care providers at both sending and receiving facilities to develop a plan for each patient's arrival; and then work with network and regional hospitals to prepare for the patient's arrival and care. 
 Logistics Role: This nurse supports patient flow in each network facility in collaboration with their local resources.  He/she acts as a traffic controller to optimize safe bed assignment and timely patient flow within each organization, and assures appropriate utilization of hospital resources. Critical thinking, collaborative decision making, and caring customer service are essential.
Transfer Back Coordinator Role: They oversees and coordinates all transfer backs and reports out at the Network Daily Pulse Check. This RN leverages network resources, delegates actual transfer backs to colleagues as needed, updates the office communication tool on status of network and regional facilities. This RN escalates all barriers to timely and efficient patient movement across the network to the RTC lead, management or the Network Physician Coordinator as appropriate. 

The position is the second level in the RTC Nurse Coordinator Clinical Ladder. 
 

EDUCATION:

Current RN licensure or compact licensure recognized by the State of Vermont is required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.
