Job Details
Join a dynamic and compassionate team who provide care for our 135 + bed facility, providing care for long-term residents as well as short term post-acute care patients who have been recently hospitalized. This provides a broad spectrum of clinical experience, including both chronic and acute illnesses and adults of all ages. We collaborate with local doctors to provide excellent care in a rewarding environment.
Our goal is to be the only SNF in the state to provide seven day per week coverage (during daytime hours). Abbreviated weekend hours once a month will translate to a full day off the following week. There is no on-call.
Day-to-day responsibilities include admitting and discharging patients, addressing acute issues as they arise, and facilitating specialty and end-of-life care as appropriate.

Requires ability to obtain DEA certificate and VT Nurse Practitioner license.
