JOB DESCRIPTION:
The Ambulatory Care Population Health Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations in the primary care settings. They apply specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. The Ambulatory Care Population Health Pharmacist Clinician will be embedded in a designated primary care clinic(s) and will be responsible to target diseases with high prevalence and healthcare cost, such as heart failure and diabetes. Through a collaborative practice agreement, the pharmacist will be responsible for the management of these disease states by providing direct patient care services and interventions, and measuring relevant outcomes. The pharmacist will also address other disease states (eg. COPD, asthma, hypertension, hypercholesterolemia, obesity, etc) as part of their clinical practice. The pharmacist will also have involvement in quality improvement, education to providers and or departments, and preceptorship in residency programs through UVMHN.
EDUCATION:
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred.
PGY2 residency in Ambulatory Care preferred.
Licensed pharmacist in the State of Vermont.
EXPERIENCE:
3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
