Intake/MDS Manager (RN) - CVPH Skilled Nursing Facility

GENERAL SUMMARY:

The SNF Intake/MDS Manager is responsible for the coordination, accuracy and of Residents Minimum Data Set (MDS).  They are responsible for assessing and coordinating the admissions process including payment accessibility and discharge planning.  They are a clinical and professional role model and demonstrates commitment to lifelong learning.  Responsibilities include compliance with regulatory standards, setting and maintain standards of nursing care and practice, coordinating services. The SNF Intake/MDS Manager works hours to meet needs of staff and organization to provide consistent visible presence on/off shifts, weekends and holidays.

QUALIFICATIONS:

• Associates in nursing required, MSN preferred.
• Current New York State Registered Nurse license required.
• 3-5 years of long-term care experience required.
• MDS certification preferred.
• Specialty certifications e.g. dementia certified, wound care certification preferred.
• Must have impeccable interpersonal skills (both verbal and written).
• Case Management experience preferred.
