The Case Management & Social Services department at Porter Medical Center is hiring for a Case Manager to join their team!

Multiple education pathways are accepted including RN, MSW, OT, PT , or a Master's Degree in a Health and Human Services field.

JOB DESCRIPTION

The Case Manager (CM) coordinates the care and services of selected patient populations across the continuum of health; promotes effective utilization and monitoring of health care resources; and assumes a critical role with the interdisciplinary team to achieve optimal patient-centered, clinical, and resource outcomes. They proactively identify and intervene to address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and other high-risk factors. They are an essential link between the patient, the healthcare team, the payer, and the community and work diligently to support their patients’ appropriate transitions of care.

EDUCATION

State of Vermont Registered Nurse (RN) license and Associate of Science in Nursing (ADN) from an accredited school of Nursing required.  The University of Vermont Health Network is moving toward an all Bachelor of Science in Nursing (BSN) workforce.  Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.  Master of Science in Nursing (MSN) preferred for this management position, however will consider BSN or ADN with commitment of continuing education upon hire.

OR

Master in Social Work (MSW) from an accredited school of Social Work required.  LICSW preferred.

OR

Master Degree from an accredited institution in a Health or Human Services field.  Licensure preferred.

OR

Bachelor of Science or higher Degree from an accredited Physical Therapy program. Licensed or eligible to be licensed to practice Physical Therapy in the State of Vermont.

OR

Bachelor of Science or higher Degree from an accredited Occupational Therapy program.  Licensed or eligible to be licensed to practice Occupational Therapy in the State of Vermont.

Case management accreditation by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications), preferred; or commitment to obtain upon hire.

EXPERIENCE

2-3 years of clinical experience in a healthcare setting (or equivalent).
