The Population Health Service Organization (PHSO) is hiring for an Outpatient Social Work Case Manager to join their team serving patients at Porter Women's Health!

  • The Pregnancy Intention Initiative serves patients by providing psychosocial screenings through interventions and referral to services for mental health, substance use, trauma, partner violence, food and housing.
  • Patients identified as at-risk in the area of mental health will work with the Outpatient Social Work Case Manager for brief intervention and counseling and/or referral to more intensive treatment as needed.
  • This position is 32 hours per week on-site at Women's Health - Porter Medical Center

JOB DESCRIPTION:

The Outpatient Social Work Case Manager (SWCM) is a comprehensive role within the interdisciplinary care team, which delivers care to patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination and chronic disease management.  The SWCM is responsible for assessment and intervention with any patient or family along the health care continuum where psychosocial and /or high risk factors are part of or interfere with the patient’s treatments or health outcomes.  SWCM utilizes advocacy skills, intervenes therapeutically and supportively, provides resources and referral information, and offers financial guidance and assistance, as needed.  The Social Work Case Manager will provide implementation, sustainment, and support of all youth, young adult and adult screening as well as brief interventions and navigation to services through the SBIRT/SBIN Programs. 

EDUCATION:

MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.

EXPERIENCE:

Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required.
