JOB DESCRIPTION:
The Per Diem Outpatient Social Work Case Manager (SWCM) plays a key role in the interdisciplinary care team, delivering longitudinal, person-centered care with a focus on prevention, health promotion, coordination, and chronic disease management. The SWCM provides psychosocial assessment and intervention for patients and families across the health care continuum, particularly when social or emotional factors impact health outcomes or treatment adherence.
This is a per diem position with no set schedule, designed to support dialysis units at the main hospital in Burlington, the South Burlington outpatient unit, the St. Albans outpatient unit, and, when needed, remotely for other UVMMC dialysis satellite units across the state. It’s a flexible, as-needed role that may include coverage during vacations, high-volume periods, or unexpected staff absences.
This role is best suited for someone who communicates clearly and directly, enjoys stepping in to meet evolving needs, and is energized by variety and unpredictability. Because the position often involves working independently, sometimes as the only social worker on-site, it requires someone with strong initiative, solid clinical judgment, and excellent organizational skills. The flexibility of the schedule means it’s important that the role fits logistically into the candidate’s life, as availability may shift based on coverage needs. This may work for someone who has another job with flexibility or someone who does not mind having some weeks with more work and others without any scheduled hours.
The SWCM uses advocacy, therapeutic and supportive intervention, and resource coordination to help patients navigate their care. Assessments, resource guidance and community referrals are also core parts of the role
EDUCATION:
MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.
EXPERIENCE:
Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required.
